Black Belt Grading Application

Some hints for applications:

• Use the current black belt application form here. (This PDF form can be filled out on screen and printed. If you own the full version of Adobe Acrobat you can also save the file.)

• Those testing for 4th degree or above must also complete this form. (excel format)
  
  
• The Instructors' comments can posted in using the normal form, or Instructors can use the on-line form in the Instructors Only Section. Alternatively the comments can be emailed directly to the Technical Director in any format.
  
  
• Make sure you check any updates to the syllabus here
  
  
• Forms can be hand written as long as the writing is legible. There is no longer a separate form called "ITF Black Belt Certificate Application Form".
  
  
• Affix one passport photo to the application form as shown. The other two passport photos, membership card and cheque must be placed in a separate small envelope and clipped to your application form. All photos must be dobok and passport style (head and shoulders) and your name clearly written on the back of each photo. Please trim each photo separately. If you print these yourself make sure you use good quality photo paper as they are used for your certificates. A 2 x 2 cm photo is no longer required.
  
  
• To be considered for Gold Points (and fee refund), please follow instructions on the forms. All credit points must have the full dates listed, must be certified by your instructor or RD, and points must be totaled.
  
  
• Don't leave your application until the last minute. It shows a lack of planning and proper preparation.
  
  
• Make sure your membership card and Instructors Report sheet have been forwarded to the ITFNZ Technical Director. Ask your instructor if this can be done well ahead of time. The Instructors report can be emailed and it is not necessary to use the supplied form.
  
  
• Do not pay by cash, cheques only please.
